Enhanced Care Planning Study
The Enhanced Care Planning (ECP) Study is a five-year R01 funded by the Agency for Healthcare Research and Quality (AHRQ) to test whether care planning helps patients to manage multiple chronic conditions. Care planning involves linking patients with a patient navigation and community health worker to access resources needed to reach their health goals. We have over 60 clinicians enrolled in the study throughout the Commonwealth of Virginia.
The study aims to help patients with uncontrolled chronic conditions, including behavioral needs, mental health needs, and social needs.

Why this matters
Behavioral factors, mental health conditions, and social needs can all affect chronic conditions, yet may not be adequately addressed by traditional medical care.
Enhanced care planning may augment traditional medical care and help improve management of chronic conditions.
This study researched an intervention to link patients with multiple uncontrolled chronic conditions with enhanced care planning.
